Healthvana provides Services that help healthcare providers engage patients and empower individuals to make better health decisions. In doing so, our goal is to be open and transparent about our Users' privacy.

This privacy policy (the "Privacy Policy") describes Healthvana's policies and procedures on the collection, use and disclosure of your information when using the Service. Your privacy is important to us and we will not share your information except as described in this Privacy Policy. Any capitalized terms not defined in this Privacy Policy have the same meaning as described in our Terms of Service. Information We Collect and Use

Healthvana collects and analyzes information to improve your experience using the Service. We use the information collected to help you access your account, store and deliver Records, facilitate communication between patients and providers, and otherwise improve patient care. We also use your information to fix technical issues, monitor metrics, ensure that Users comply with our Terms of Service and applicable laws, and generally understand how the Service is used.

Because the Services is designed to help patients and providers improve patient care, we may receive your name, email address, phone number, date of birth, password, signature, Records, and other personally identifiable information (collectively, the "Personal Information") to help you use the Service. Any Personal Information provided by you, health care provider(s), or testing service(s) will not be shared via the Service unless you choose to do so.

Cookies are required to use the Healthvana service. A cookie is a small data file, which often includes an anonymous unique identifier that is sent to your browser from a web site's computers and stored on your computer's hard drive. Healthvana uses two types of cookies: Session Cookies, and Persistent Cookies. Session Cookies are temporary cookies that remain in the cookie file of your browser until you leave the website. Session cookies do not store any personally identifying information, only a unique visitor ID number that we can use to customize the Healthvana functionality for you. Persistent Cookies remain in the cookie file of your browser for much longer, even after you leave the Healthvana website. Persistent cookies also do not store any Personal Information, only preferences that should persist from visit to visit, like "Remember Me."

Healthvana's servers automatically record information ("Log Files") created by your use of the Service. Log Files may include information such as your IP address, browser type, the referring domain, pages visited, and search terms. Other actions may also be included in Log Files.

To create the best possible Service and functionality for Users, Healthvana may review your Personal Information internally as needed. Healthvana may use your Personal Information to help you manage your Records or otherwise as directed by you in the course of providing the Service. We may also provide your Personal Information to approved vendors for their use in assisting us with the maintenance and operation of the Service, including for disseminating Healthvana's email and text messaging communications. For example, if we reasonably believe that one of our Users has committed identity theft, we will report such User to the proper law enforcement agency. If we are legally compelled to disclose your Personal Information to a third party, we will attempt to notify you by sending an email to the email address in our records unless doing so would violate the law or unless you have not provided your correct email address to us.

Healthvana may share or disclose your non-private, aggregated or otherwise non-personal information (such as anonymous usage data, URLs, platform types, number of clicks, etc.) with third parties to help them understand trends or other information related to the use of the Service.

Other Security Issues

Healthvana takes commercially reasonable technical precautions to help keep data secure. We strive to maintain the reliability, accuracy, completeness and currency of our Users' Personal Information in our databases. We also strive to protect the privacy and security of our databases and the data we collect. The Personal Information data we collect is based solely on information provided by our Users or their health care providers. We cannot guarantee the authenticity of any data that Users may provide about themselves.

The security measures in place on the Service and computer systems where this information is stored aim to protect the loss, misuse or alteration of the information you provide to us. Healthvana is not responsible for your handling, sharing, re-sharing and/or distribution of your Personal Information. Moreover, if you send your Records or other Personal Information (electronically or in any other manner) to another party or request that Healthvana send your Personal Information to another party, Healthvana is not responsible for any harm or other consequences from third party use or re-sharing of your Personal Information.

While using the Service, you can be directed through links to other websites that are beyond Healthvana's control. Healthvana is not responsible for the privacy practices and policies of such websites.

Maintenance of Accurate Information

We have established procedures to correct inaccurate information in a timely manner.
